Benicar (olmesartan medoxomil) is an angiotensin II receptor blocker (ARB) prescribed for the management of hypertension in adults. It works by blocking the action of certain natural substances that tighten blood vessels, allowing blood to flow more smoothly and the heart to pump more efficiently. This medication is trusted by healthcare professionals for its efficacy in lowering high blood pressure, thereby helping to reduce the risk of strokes, heart attacks, and kidney problems. Consistent use as directed can contribute significantly to long-term cardiovascular wellness.

Features

  • Active ingredient: Olmesartan medoxomil
  • Drug class: Angiotensin II receptor blocker (ARB)
  • Available in tablet form: 5 mg, 20 mg, 40 mg strengths
  • Once-daily dosing for patient convenience
  • FDA-approved for the treatment of hypertension
  • May be used as monotherapy or in combination with other antihypertensive agents

Benefits

  • Effectively lowers systolic and diastolic blood pressure
  • Reduces the risk of cardiovascular events such as stroke and myocardial infarction
  • Helps protect kidney function in patients with hypertension and type 2 diabetes
  • Supports long-term management of high blood pressure with once-daily dosing
  • Generally well-tolerated with a favorable side effect profile for many patients
  • Does not usually cause coughing, a common side effect of ACE inhibitors

Common use

Benicar is primarily indicated for the treatment of hypertension in adult patients. It may be used alone or in combination with other antihypertensive medications. In some cases, it is also utilized off-label for certain cardiovascular conditions under specialist supervision, though such use should always be guided by a healthcare provider’s assessment of individual patient needs and risks.

Dosage and direction

The recommended starting dose of Benicar is 20 mg once daily. Depending on the patient’s blood pressure response, the dosage may be increased to a maximum of 40 mg once daily after two weeks of therapy. For patients with possible depletion of intravascular volume, such as those treated with diuretics, a lower starting dose of 5 mg may be considered. Benicar may be taken with or without food. Tablets should be swallowed whole with a glass of water. Dosage adjustments may be necessary for patients with hepatic impairment.

Precautions

Patients should be monitored for hypotension, especially after initiation or dosage increase. Renal function should be assessed periodically, particularly in patients with renal artery stenosis, heart failure, or volume depletion. Use during pregnancy can cause injury or death to the developing fetus; discontinue immediately if pregnancy is detected. Caution is advised in patients with significant aortic or mitral valve stenosis or obstructive hypertrophic cardiomyopathy. Electrolyte imbalances, particularly hyperkalemia, should be monitored, especially in patients with renal impairment or diabetes.

Contraindications

Benicar is contraindicated in patients with known hypersensitivity to olmesartan medoxomil or any component of the formulation. It is contraindicated in pregnancy due to the risk of fetal harm. Concomitant use with aliskiren in patients with diabetes is also contraindicated. Patients with a history of angioedema related to previous ARB or ACE inhibitor treatment should not use Benicar.

Possible side effect

Common side effects may include dizziness, headache, diarrhea, upper respiratory tract infection, and back pain. Less frequently reported side effects include hyperglycemia, hypertriglyceridemia, influenza-like symptoms, pharyngitis, rhinitis, sinusitis, and gastroenteritis. Rare but serious side effects can include severe hypotension, acute renal failure, hyperkalemia, and sprue-like enteropathy characterized by severe, chronic diarrhea with substantial weight loss.

Drug interaction

Benicar may interact with other medications that increase potassium levels, such as potassium supplements, salt substitutes containing potassium, or potassium-sparing diuretics. Concurrent use with NSAIDs may lead to reduced antihypertensive effects and increased risk of renal impairment. Lithium levels may increase when co-administered with Benicar. Use with other antihypertensive agents may have additive effects. Aliskiren should not be used concomitantly in patients with diabetes.

Missed dose

If a dose is missed, it should be taken as soon as remembered on the same day. If it is almost time for the next scheduled dose, the missed dose should be skipped and the regular dosing schedule resumed. Patients should not take a double dose to make up for a missed one. Consistency in dosing is important for maintaining stable blood pressure control.

Overdose

Symptoms of overdose may include hypotension and tachycardia. Bradycardia could occur with parasympathetic (vagal) stimulation. If overdose occurs, the patient should be placed in supine position and given supportive treatment, including intravenous normal saline solution. Olmesartan is not removed by hemodialysis. Close monitoring of blood pressure and heart rate is essential, with appropriate symptomatic treatment as necessary.

Storage

Store Benicar tablets at room temperature between 20°C to 25°C (68°F to 77°F), with excursions permitted between 15°C to 30°C (59°F to 86°F). Keep in the original container, tightly closed, and protect from moisture and light. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging.

Reviews

Clinical studies have demonstrated Benicar’s efficacy in reducing blood pressure, with many patients achieving target levels. Healthcare providers often note its generally favorable tolerability profile compared to some other antihypertensive classes. Some patients report satisfactory blood pressure control with minimal side effects, though individual experiences may vary. Post-marketing surveillance continues to monitor its safety profile in diverse patient populations.
