Common use

Ciprodex Ophthalmic Solution is indicated for the treatment of superficial ocular infections, such as bacterial conjunctivitis, caused by susceptible strains of organisms. It is also used in cases where corticosteroid use is indicated for inflammatory conditions and where a superficial bacterial ocular infection or a risk of bacterial ocular infection exists. Its use should be reserved for cases where the anti-inflammatory action of dexamethasone is deemed necessary to reduce swelling, redness, and discomfort.

Dosage and direction

The recommended dosage for Ciprodex Ophthalmic Solution is one to two drops instilled into the conjunctival sac of the affected eye(s) every 12 hours, for 7 days, or as directed by a healthcare professional. Before administration, the bottle should be shaken well to ensure a homogeneous suspension. To avoid contamination, care should be taken not to touch the dropper tip to any surface, including the eye or fingers. If more than one topical ophthalmic drug is being used, the solutions should be administered at least 5 minutes apart.

Precautions

  • Prolonged use may result in overgrowth of non-susceptible organisms, including fungi. If superinfection occurs, appropriate therapy should be initiated.
  • Use with caution in patients with a history of sensitivity to any component of the preparation.
  • Corneal perforation has been reported in the use of topical steroids in diseases causing thinning of the cornea or sclera.
  • Intraocular pressure should be routinely monitored even though it may be difficult in children and uncooperative patients.
  • Not for injection or direct introduction into the anterior chamber of the eye.
  • Patients should be advised not to wear contact lenses if they have signs and symptoms of ocular infection.

Contraindications

Ciprodex Ophthalmic Solution is contraindicated in patients with known hypersensitivity to any of its components, to other quinolones, or to any of the other ingredients in the formulation. It is also contraindicated in most viral diseases of the cornea and conjunctiva, including epithelial herpes simplex keratitis (dendritic keratitis), vaccinia, and varicella, and also in mycobacterial infection of the eye and fungal diseases of ocular structures.

Possible side effect

Common adverse reactions occurring in approximately 1-5% of patients include conjunctival hyperemia, blurred vision, corneal deposits, ocular irritation, ocular pain, and taste disturbance. Less frequently, allergic reactions, eyelid edema, photophobia, and tearing may occur. As with other antibiotic-steroid combinations, prolonged use may lead to secondary infection, increased intraocular pressure, glaucoma, optic nerve damage, cataract formation, and delayed wound healing.

Drug interaction

Although systemic absorption is low following topical ocular administration, interactions are possible. Concurrent use with other corticosteroid-containing products may increase the risk of systemic steroid effects. No specific drug interaction studies have been conducted with Ciprodex. However, caution is advised when using with other medications that are known to prolong the QT interval. Patients should inform their physician of all medications they are taking.

Missed dose

If a dose is missed, it should be administered as soon as possible. However, if it is almost time for the next dose, the missed dose should be skipped, and the regular dosing schedule resumed. Doubling the dose is not recommended.

Overdose

Topical overdose of Ciprodex Ophthalmic Solution is unlikely to produce significant systemic effects due to limited absorption. However, the eye may be flushed with warm tap water in case of accidental excessive application. In case of accidental ingestion, symptomatic and supportive measures should be taken. There is no specific antidote.

Storage

Store at controlled room temperature 15°-30°C (59°-86°F). Protect from light and freezing. Keep the bottle tightly closed when not in use. Discard any unused portion 28 days after opening to avoid contamination and reduced efficacy.
