Common use

Diarex is commonly used for the symptomatic treatment of acute, non-specific diarrhea in individuals aged 6 years and above. It is particularly effective in cases of traveler’s diarrhea, viral gastroenteritis, and mild foodborne illness. The medication works by slowing intestinal motility, allowing for increased water absorption and more formed stools. It is not intended for chronic diarrhea conditions or those caused by specific bacterial infections requiring antibiotic therapy. Patients should consult a healthcare provider if diarrhea persists beyond 48 hours or is accompanied by high fever or blood in stool.

Dosage and direction

For adults and children 12 years and older: The initial dose is 4 mg (two tablets or 20 ml of liquid) after the first loose stool, followed by 2 mg (one tablet or 10 ml) after each subsequent loose stool. The maximum daily dose should not exceed 8 mg. For children 6-11 years: Consult a physician for appropriate dosing based on weight; typically 1 mg after each loose stool, not to exceed 3 mg in 24 hours. Diarex should be taken with water and may be taken with or without food. Do not use for more than 2 days unless directed by a healthcare professional.

Precautions

Do not use if you have a fever or if there is blood in your stool. Avoid using in children under 6 years unless specifically advised by a pediatrician. Use with caution in elderly patients due to increased sensitivity to antimotility effects. Those with liver impairment should consult a healthcare provider before use. Diarex may cause drowsiness or dizziness; avoid operating machinery or driving until you know how it affects you. Keep out of reach of children to prevent accidental ingestion.

Contraindications

Diarex is contraindicated in patients with known hypersensitivity to loperamide hydrochloride or any other component of the formulation. It should not be used in patients with abdominal pain without diarrhea, acute dysentery (characterized by blood in stools and high fever), or pseudomembranous colitis associated with broad-spectrum antibiotics. Avoid use in patients with severe hepatic impairment or those taking medications that strongly inhibit CYP3A4 enzymes. Contraindicated in children under 2 years due to risk of serious respiratory depression.

Possible side effect

Common side effects may include constipation, dizziness, drowsiness, nausea, and abdominal discomfort. Less frequently, patients may experience dry mouth, vomiting, or skin rash. In rare cases, serious side effects such as severe abdominal distension, paralytic ileus, or allergic reactions including anaphylaxis may occur. Discontinue use and seek medical attention if you experience rapid heart rate, fainting, or difficulty breathing. Most side effects are dose-dependent and typically resolve upon discontinuation of the medication.

Drug interaction

Diarex may interact with other medications that cause constipation or central nervous system depression, such as opioids, sedatives, or tranquilizers. Concurrent use with QT-prolonging agents may increase the risk of cardiac arrhythmias. Strong inhibitors of CYP3A4 (e.g., ketoconazole, ritonavir) and P-glycoprotein inhibitors may significantly increase loperamide plasma concentrations. Avoid concomitant use with adsorbents like activated charcoal, which may reduce its effectiveness. Always inform your healthcare provider about all medications you are taking, including prescription, over-the-counter, and herbal products.

Missed dose

Diarex is taken as needed after loose stools rather than on a fixed schedule. If you miss a dose after a loose stool, take it as soon as you remember unless it is nearly time for another loose stool occurrence. Do not double the dose to make up for a missed one. The medication should only be taken after episodes of diarrhea, not proactively at regular intervals.

Overdose

In case of overdose, symptoms may include severe constipation, central nervous system depression (including drowsiness, lethargy, and respiratory depression), and urinary retention. In severe cases, QT prolongation and cardiac arrhythmias may occur. Seek immediate medical attention or contact a poison control center. Treatment is supportive and may include activated charcoal if administered soon after ingestion. Naloxone may be used to reverse opioid-like effects in severe cases, though multiple doses may be required due to loperamide’s long half-life.

Storage

Store Diarex at room temperature between 15-30°C (59-86°F). Keep the container tightly closed and protect from moisture and light. Do not freeze the liquid suspension. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging. Properly discard any unused medication after the treatment course is completed.
