Nizoral (ketoconazole) is indicated for the topical treatment of fungal infections caused by susceptible organisms. Its primary uses include the treatment of tinea corporis (ringworm of the body), tinea cruris (jock itch), tinea pedis (athlete’s foot), pityriasis versicolor, and cutaneous candidiasis when formulated as a cream. The shampoo formulation is specifically indicated for the treatment of seborrheic dermatitis of the scalp and dandruff caused by the yeast Malassezia furfur.

Dosage and direction

For Topical Cream (2%): Apply a sufficient amount of Nizoral cream to cover the affected and immediate surrounding area(s) once or twice daily. Treatment duration is typically 2 to 4 weeks for most dermatophyte infections and 2 weeks for cutaneous candidiasis. For pityriasis versicolor, application once daily for 2 weeks is generally sufficient. Wash hands before and after application, unless the hands are the treated area.

For Shampoo (2%): For the treatment of seborrheic dermatitis: Use twice weekly for 4 weeks, allowing a minimum of 3 days between applications. Wet hair thoroughly, apply approximately 5 ml of shampoo, massage into a lather, and leave on the scalp for 5 minutes before rinsing thoroughly. For the treatment of dandruff: Use twice weekly to control symptoms. For maintenance therapy, use once every 1-2 weeks or as directed by a physician.

Precautions

  • Nizoral is for external use only. Avoid contact with the eyes, nose, mouth, and other mucous membranes. If contact occurs, rinse thoroughly with water.
  • Do not use on large areas of broken or damaged skin, as increased systemic absorption may occur.
  • Use with caution in patients with known hypersensitivity to other imidazole antifungals, due to potential for cross-sensitivity.
  • If a reaction suggesting sensitivity or chemical irritation should occur, use of the medication should be discontinued.
  • The safety and efficacy of use in pediatric patients under the age of 12 have not been fully established for all indications; use under medical supervision.

Contraindications

Nizoral topical formulations are contraindicated in patients with a known hypersensitivity to ketoconazole, any other imidazole antifungal, or to any of the excipients listed in the formulation.

Possible side effect

Most side effects are local and mild. The most commonly reported adverse reactions are local burning, itching, stinging, and irritation at the application site. Less frequently, contact dermatitis, erythema, and dry skin have been reported. Oiliness or dryness of the hair and scalp may occur with the shampoo formulation. Missed dose

If a dose of Nizoral cream or shampoo is missed, apply it as soon as it is remembered. If it is almost time for the next scheduled application, skip the missed dose and resume the usual dosing schedule. Do not apply a double dose to make up for a missed one.

Overdose

Topical overdose of Nizoral is unlikely due to its limited application area. However, accidental ingestion of the topical cream or shampoo requires immediate medical attention. Symptoms of ketoconazole overdose following ingestion may include nausea, vomiting, diarrhea, and abdominal pain. Treatment should be supportive and symptomatic.

Storage

Store Nizoral cream and shampoo at room temperature, between 15°C and 30°C (59°F and 86°F). Do not freeze. Keep the tube or bottle tightly closed and out of direct sunlight. Keep all medicines out of the reach of children and pets. Do not use after the expiration date printed on the packaging.
