Dosage and direction
Apply a pea-sized amount to the entire affected area once daily, in the evening, on thoroughly dry skin. Using more than recommended will not increase efficacy and will significantly increase the risk of irritation. After cleansing the face with a gentle, non-abrasive cleanser, wait 20-30 minutes for the skin to dry completely before application. Spread the product evenly, avoiding the immediate eye area (eyelids), creases of the nose, and the corners of the mouth.
Precautions
- For external use only. Avoid contact with eyes, eyelids, lips, and mucous membranes.
- This medication increases skin sensitivity to sunlight. Sun avoidance, protective clothing, and a broad-spectrum sunscreen (SPF 30 or higher) are mandatory during use and for some time after discontinuation.
- Extreme weather conditions, such as wind and cold, can exacerbate skin irritation.
- Do not use on sunburned, windburned, dry, chapped, or broken skin.
- Delay the use of other potentially irritating topical products (e.g., astringents, abrasives, high-percentage AHAs/BHAs, products with alcohol or spices) until the skin has acclimated to tretinoin.
- “Purging,” a temporary initial flare-up of acne, is common as impactions are accelerated to the surface.
Contraindications
This product is contraindicated in individuals with a known hypersensitivity to tretinoin or any other component of the formulation. Use is also contraindicated in cases of personal or familial history of cutaneous epitheliomas.
Possible side effect
The most common side effects are localized to the site of application and are often signs of the medication’s pharmacologic activity. These are usually dose-dependent and tend to diminish with continued use.
- Very Common (>10%): Skin dryness, scaling, flaking, redness (erythema), burning sensation, stinging, warmth.
- Common (1-10%): Peeling, itching (pruritus), skin pain or irritation, severe redness.
- Uncommon (0.1-1%): Temporary hyper- or hypopigmentation, blistering, crusting, swelling (edema).
- Rare (<0.1%): Allergic contact dermatitis.
Drug interaction
Concurrent use of other topical medications with a high potential for irritation (e.g., benzoyl peroxide, salicylic acid, resorcinol, sulfur) should be approached with caution and under medical supervision to avoid cumulative irritation. It is generally advised to apply them at different times of the day. Concomitant use of topical antibiotics like clindamycin is common and often synergistic. Photosensitizing drugs (e.g., thiazides, tetracyclines, fluoroquinolones, phenothiazines, sulfonamides) can increase the risk of phototoxicity.
Missed dose
If a dose is missed, apply it as soon as remembered on that same evening. If it is nearly time for the next application, skip the missed dose and resume the regular schedule. Do not apply a double dose to compensate for a missed one.
Overdose
Topical overdose may lead to severe erythema, peeling, burning, and discomfort. Acute oral ingestion may lead to the same side effects associated with excessive oral intake of Vitamin A (hypervitaminosis A), including headache, dizziness, lethargy, vomiting, and peeling of the skin. In case of accidental ingestion, seek immediate medical attention or contact a poison control center.
Storage
Store at controlled room temperature (20°C to 25°C or 68°F to 77°F). Do not freeze. Keep the tube tightly closed and protect from light. Keep out of reach of children and pets.
