Common use

Rizact is commonly used for the acute treatment of migraine attacks in adults. It is specifically indicated for migraines with or without aura. It is not to be used for preventing migraines or for treating other types of headaches, such as tension headaches or cluster headaches. Dosage and direction

The recommended dose is 5 mg or 10 mg taken as a single dose at the onset of migraine symptoms. If the migraine returns or if there is only partial response, a second dose may be taken after at least 2 hours, not to exceed 30 mg in any 24-hour period. The orally disintegrating tablet should be placed on the tongue and allowed to dissolve, then swallowed with saliva—no liquid is needed. The standard tablet should be swallowed whole with water. Dosage adjustment may be necessary for patients on MAO inhibitors or with hepatic impairment. The maximum recommended single dose is 10 mg.

Precautions

Patients should be carefully evaluated for cardiovascular status before prescribing, as triptans may cause coronary vasospasm. Use with caution in patients with risk factors for coronary artery disease unless cardiovascular evaluation provides satisfactory results. May cause sensations of tightness, pain, or pressure in the chest, throat, neck, or jaw—typically non-cardiac but requires evaluation if severe or concerning. Monitor for serotonin syndrome, particularly when used with other serotonergic drugs. Use cautiously in patients with hepatic impairment, pregnancy, or breastfeeding. May cause dizziness or drowsiness—caution when operating machinery. Not recommended for use in patients with basilar or hemiplegic migraine.

Contraindications

Rizact is contraindicated in patients with ischemic heart disease, history of myocardial infarction, or documented silent ischemia. Contraindicated in patients with coronary artery vasospasm, including Prinzmetal’s angina. Should not be used in patients with uncontrolled hypertension. Contraindicated in patients with cerebrovascular syndromes including strokes and transient ischemic attacks. Not to be used in patients with peripheral vascular disease. Contraindicated within 24 hours of treatment with another triptan or ergotamine-containing medication. Contraindicated in patients with severe hepatic impairment. Should not be used in patients with known hypersensitivity to rizatriptan or any component of the formulation.

Possible side effects

Most common side effects include dizziness, somnolence, fatigue, nausea, and chest/neck/jaw tightness or pain. Less frequently reported effects include palpitations, tachycardia, hypertension, dry mouth, diarrhea, and muscle weakness. Rare but serious side effects may include coronary artery vasospasm, myocardial ischemia, myocardial infarction, ventricular tachycardia, ventricular fibrillation, cerebrovascular events, gastrointestinal ischemic events, and serotonin syndrome. Some patients may experience sensations of tingling, heat, or cold.

Drug interaction

Significant interactions occur with MAO inhibitors—concurrent use contraindicated (wait至少 2 weeks after discontinuing MAOI). Ergot-containing medications may cause prolonged vasospastic reactions—avoid within 24 hours of Rizact use. Other triptans should not be used within 24 hours of Rizact administration. Propranolol increases rizatriptan concentrations—use 5 mg dose with maximum daily dose of 10 mg. Selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) may increase risk of serotonin syndrome. Oral contraceptives may slightly decrease rizatriptan clearance.

Missed dose

Rizact is taken on an as-needed basis for acute migraine attacks and does not follow a regular dosing schedule. Therefore, the concept of a “missed dose” does not apply. Patients should take Rizact at the onset of migraine symptoms. If a dose is not taken during a migraine attack, simply take it when needed for the next migraine episode, following the same dosing instructions.

Overdose

Symptoms of overdose may include dizziness, drowsiness, fainting, tachycardia, bradycardia, hypotension, hypertension, seizure, and respiratory depression. In cases of suspected overdose, provide supportive care including continuous ECG monitoring for at least 20 hours. Ensure adequate airway and ventilation. Hemodialysis is unlikely to be beneficial due to extensive protein binding. Contact a poison control center for latest guidance on management.

Storage

Store at room temperature between 15-30°C (59-86°F). Protect from light and moisture. Keep orally disintegrating tablets in the original blister package until time of use. Keep all medications out of reach of children and pets. Do not use after the expiration date printed on the packaging. Properly discard any unused medication that is no longer needed or has expired.
