Common use

Sinemet is primarily indicated for the treatment of the symptoms of idiopathic Parkinson’s disease (paralysis agitans), postencephalitic parkinsonism, and symptomatic parkinsonism which may follow injury to the nervous system by carbon monoxide intoxication or manganese intoxication. It is not indicated for the treatment of drug-induced extrapyramidal reactions.

Dosage and direction

Dosage is highly individualized and must be titrated according to the patient’s response. Therapy is typically initiated with a low dose, such as one tablet of Sinemet 25/100 (containing 25 mg carbidopa and 100 mg levodopa) taken three times daily. The dose may be increased by one tablet every day or every other day until a maximum therapeutic response is achieved. The frequency of dosing usually ranges from three to four times daily, though some patients with advanced disease and motor complications may require more frequent administration. Tablets should be swallowed whole with water and can be taken with or without food, though a protein-rich meal may interfere with absorption. The controlled-release formulation (Sinemet CR) is typically administered on a less frequent schedule, often twice daily.

Precautions

  • Patients should be monitored for the development of depression with concomitant suicidal tendencies.
  • Use with caution in patients with severe cardiovascular or pulmonary disease, bronchial asthma, renal, hepatic, or endocrine disease.
  • May cause dizziness or orthostatic hypotension; caution patients against rising rapidly from a sitting or lying position.
  • Can cause drowsiness and somnolence; advise patients about the potential of falling asleep during activities of daily living, including operating machinery or driving.
  • May elevate blood urea nitrogen, creatinine, uric acid, and transaminases.
  • Periodic evaluations of hepatic, hematopoietic, cardiovascular, and renal function are recommended during extended therapy.
  • Not recommended for patients under 18 years of age.

Contraindications

  • Hypersensitivity to any component of this medication.
  • Concomitant use with nonselective monoamine oxidase (MAO) inhibitors or within 2 weeks of discontinuing them. MAO-B inhibitors (e.g., rasagiline, selegiline) may be used with caution.
  • Patients with narrow-angle glaucoma.
  • Patients with a history of melanoma or with undiagnosed skin lesions.

Possible side effect

Common side effects may include: nausea, vomiting, dizziness, orthostatic hypotension, arrhythmias, choreiform, dystonic, and other abnormal involuntary movements. Psychiatric disturbances such as agitation, anxiety, confusion, depression, hallucinations, and paranoid ideation can occur. Other effects include dry mouth, dysphagia, darkening of sweat and urine, and on-off phenomena.

Drug interaction

  • Antihypertensive Drugs: May produce orthostatic hypotension, requiring dosage adjustment of the antihypertensive agent.
  • Phenothiazines and Butyrophenones: These antipsychotic drugs can reduce the therapeutic effects of Sinemet.
  • Tricyclic Antidepressants: May reduce the absorption of levodopa and increase the risk of hypertension and dyskinesia.
  • MAO Inhibitors: Contraindicated due to risk of hypertensive crisis.
  • Dopamine D2 Receptor Antagonists (e.g., metoclopramide, risperidone): May diminish the effectiveness of Sinemet.
  • Anticholinergics: May enhance the therapeutic effects of levodopa but also increase the risk of adverse effects.
  • Protein-rich meals: Can interfere with the gastrointestinal absorption of levodopa.

Missed dose

If a dose is missed, it should be taken as soon as it is remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped. The patient should not double the next dose to make up for the missed one, as this increases the risk of side effects.

Overdose

Symptoms of overdose are primarily related to the levodopa component and may include severe nausea and vomiting, profound hypotension, cardiac arrhythmias, and intense abnormal involuntary movements. Mental disturbances including confusion, insomnia, and agitation are common. In severe cases, psychosis may occur. Management is primarily symptomatic and supportive. Pyridoxine (Vitamin B6) is NOT effective in reversing the effects of overdose of Sinemet due to the presence of the decarboxylase inhibitor carbidopa. Gastric lavage may be considered with appropriate airway protection. ECG monitoring is recommended.

Storage

Store at room temperature between 20°C to 25°C (68°F to 77°F). Excursions are permitted between 15°C and 30°C (59°F and 86°F). Keep the container tightly closed to protect from moisture and light. Keep out of reach of children and pets.
