Tetracycline is a time-tested, broad-spectrum antibiotic belonging to the tetracycline class, widely utilized in clinical practice for its efficacy against a diverse range of bacterial pathogens. It functions by inhibiting protein synthesis at the ribosomal level, thereby halting bacterial growth and proliferation. This agent is indicated for both common and complex infections, offering clinicians a versatile therapeutic option. Proper usage under medical supervision ensures optimal outcomes while minimizing resistance development.
Features
- Active ingredient: Tetracycline hydrochloride
- Available in 250 mg and 500 mg oral capsules
- Broad-spectrum activity against Gram-positive and Gram-negative bacteria
- Inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit
- Standard blister packaging with moisture-resistant coating
Benefits
- Effectively treats respiratory, urinary, skin, and sexually transmitted infections
- Reduces bacterial load and associated inflammatory symptoms
- Helps prevent complications from susceptible bacterial infections
- Supports combination therapy regimens for enhanced coverage
- Cost-effective option for long-term or prophylactic use
Common use
Tetracycline is commonly prescribed for infections caused by susceptible strains of bacteria, including acne vulgaris, bronchitis, pneumonia, urinary tract infections, and certain sexually transmitted diseases such as chlamydia. It is also used in the management of Helicobacter pylori infections as part of combination therapy and for prophylaxis in areas endemic to malaria. Off-label uses may include treatment for anthrax exposure and certain rickettsial infections.
Dosage and direction
The typical adult dosage is 500 mg twice daily or 250 mg four times daily, depending on infection severity. For severe infections, higher doses may be prescribed under close medical supervision. Administration should occur on an empty stomach, at least 1 hour before or 2 hours after meals, with a full glass of water to enhance absorption and reduce esophageal irritation. Dosage adjustments are necessary for patients with renal impairment. Treatment duration varies by indication but typically ranges from 7 to 14 days.
Precautions
Avoid dairy products, antacids, or iron supplements within 2–3 hours of dosing, as they can significantly reduce absorption. Use with caution in patients with hepatic or renal dysfunction. Prolonged use may result in fungal or bacterial superinfection, including colitis. Tetracycline can cause photosensitivity; advise patients to use sunscreen and protective clothing. Not recommended during tooth development (last half of pregnancy, infancy, childhood up to age 8) due to risk of permanent tooth discoloration.
Contraindications
Hypersensitivity to tetracycline or any component of the formulation. Contraindicated in patients with severe hepatic impairment. Do not use in children under 8 years of age, during pregnancy, or while breastfeeding due to risks of tooth discoloration and inhibited bone growth. Avoid in patients with systemic lupus erythematosus due to potential exacerbation.
Possible side effect
Common side effects include nausea, vomiting, diarrhea, and epigastric distress. Photosensitivity reactions, manifesting as exaggerated sunburn, may occur. Less frequently, glossitis, stomatitis, black hairy tongue, or esophageal ulceration can develop. Rare but serious effects include pseudotumor cerebri, hepatotoxicity, and blood dyscrasias. Discoloration of teeth and enamel hypoplasia may occur if used during tooth development.
Drug interaction
Tetracycline may interact with antacids containing aluminum, calcium, or magnesium; iron preparations; and bismuth subsalicylate, reducing its absorption. Concurrent use with oral contraceptives may decrease contraceptive efficacy. Potentiates the effect of warfarin, increasing risk of bleeding. May increase methotrexate toxicity. Avoid concomitant use with retinoids due to increased risk of benign intracranial hypertension.
Missed dose
If a dose is missed, take it as soon as remembered unless it is nearly time for the next dose. Do not double the dose to catch up. Maintaining consistent serum levels is important for efficacy, so try to adhere to the prescribed schedule. If multiple doses are missed, consult a healthcare provider for guidance.
Overdose
Symptoms of overdose may include nausea, vomiting, and diarrhea. In severe cases, pancreatitis or hepatotoxicity can occur. Management is supportive; there is no specific antidote. Gastric lavage may be considered if ingestion was recent. Hemodialysis is not effective for removal. Seek immediate medical attention or contact a poison control center.
Storage
Store at room temperature (15–30°C or 59–86°F) in a tightly closed container, protected from light and moisture. Keep out of reach of children and pets. Do not use after the expiration date printed on the packaging. Avoid storage in bathrooms or damp areas.
