The Tiova Inhaler is a prescription medication designed for the maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema. It contains the active ingredient Tiotropium Bromide, a long-acting muscarinic antagonist (LAMA) that works by relaxing the muscles around the airways, helping to keep them open and improve breathing. This inhaler is intended for regular daily use to control symptoms, reduce exacerbations, and enhance overall lung function and quality of life in patients with COPD. It is not indicated for the immediate relief of acute bronchospasm.

Features

  • Active ingredient: Tiotropium Bromide (18 mcg per inhalation)
  • Delivery system: Breath-activated, dry powder inhaler (DPI)
  • Mechanism of action: Long-acting muscarinic antagonist (LAMA)
  • Dosage form: Metered dose, pre-measured capsules for use with the HandiHaler device
  • Duration of action: Provides 24-hour bronchodilation with once-daily dosing
  • Prescription status: Available only with a valid medical prescription

Benefits

  • Sustained bronchodilation for 24 hours, ensuring all-day symptom control and reduced breathlessness
  • Decreases the frequency and severity of COPD exacerbations, potentially reducing hospitalizations
  • Improves exercise tolerance and overall quality of life by enhancing lung function
  • Simple once-daily dosing regimen promotes adherence and convenience for patients
  • Helps reduce dynamic hyperinflation, allowing for more effective emptying of the lungs
  • Compatible with other maintenance therapies, supporting comprehensive COPD management plans

Common use

The Tiova Inhaler is commonly prescribed for the long-term, once-daily maintenance treatment of airflow obstruction in patients with chronic obstructive pulmonary disease (COPD). It is used to manage symptoms such as wheezing, shortness of breath, and chronic coughing. It is not a rescue inhaler and should not be used for the immediate relief of sudden breathing problems. Patients typically use it as part of a broader COPD management strategy that may include other bronchodilators, corticosteroids, pulmonary rehabilitation, and lifestyle modifications.

Dosage and direction

The recommended dosage for the Tiova Inhaler is the inhalation of the contents of one capsule (18 mcg Tiotropium Bromide) once daily, at the same time each day, using the HandiHaler device. Do not swallow the capsule; it is for inhalation only. To use, open the blister pack, place the capsule in the center chamber of the HandiHaler, pierce the capsule by firmly pressing the side buttons once, exhale fully away from the mouthpiece, place lips tightly around the mouthpiece, and inhale slowly and deeply until the lungs are full. Hold your breath for up to 10 seconds, then exhale slowly. Repeat inhalation if any powder remains. Always close the device after use and discard the empty capsule. Do not use more than one inhalation per day.

Precautions

  • Not for use in the treatment of acute bronchospasm; always have a fast-acting rescue inhaler available.
  • Use with caution in patients with narrow-angle glaucoma, prostatic hyperplasia, or bladder neck obstruction, as anticholinergic effects may exacerbate these conditions.
  • May cause paradoxical bronchospasm; if this occurs, discontinue use immediately and seek alternative therapy.
  • Avoid getting the powder in the eyes, as it may cause blurring of vision or pupil dilation.
  • Not recommended for use in children or adolescents under 18 years of age.
  • Use during pregnancy or breastfeeding only if clearly needed and under medical supervision, as safety data are limited.
  • Regular monitoring of lung function and symptom control is advised to assess therapeutic response.

Contraindications

  • Hypersensitivity to Tiotropium Bromide, atropine or its derivatives, or any component of the formulation.
  • Patients with a history of hypersensitivity reactions such as rash, urticaria, or angioedema to similar anticholinergic agents.
  • Not indicated for the initial treatment of acute episodes of bronchospasm.

Possible side effect

Common side effects may include dry mouth, constipation, and upper respiratory tract infection. Other possible side effects are:

  • Sinusitis, pharyngitis, rhinitis
  • Dyspepsia, abdominal pain
  • Urinary retention (especially in patients with pre-existing prostatic hyperplasia)
  • Tachycardia, palpitations
  • blurred vision, glaucoma exacerbation (if powder comes in contact with eyes)
  • Cough, hoarseness
  • Dizziness, headache
  • Allergic reactions such as skin rash or itching
  • Paradoxical bronchospasm (wheezing immediately after inhalation)

Drug interaction

Tiotropium Bromide may interact with other anticholinergic drugs (e.g., ipratropium, aclidinium), potentially increasing the risk of side effects such as dry mouth, urinary retention, or constipation. Concurrent use with beta-adrenergic agonists (e.g., salmeterol, formoterol) is common in COPD management but should be monitored. Use caution with drugs that are excreted renally, as Tiotropium is primarily eliminated via the kidneys. No significant interactions with cytochrome P450 substrates are known, but always inform your healthcare provider of all medications, including over-the-counter drugs and supplements.

Missed dose

If a dose is missed, it should be taken as soon as remembered on the same day. However, if it is almost time for the next scheduled dose, skip the missed dose and resume the regular dosing schedule. Do not double the dose to make up for a missed one, as this may increase the risk of side effects.

Overdose

Overdose may lead to exaggerated anticholinergic effects such as severe dry mouth, visual disturbances, tachycardia, urinary retention, constipation, and possible confusion. There is no specific antidote for Tiotropium Bromide overdose. Treatment is supportive and symptomatic. In case of suspected overdose, seek immediate medical attention. Dialysis is not likely to be effective due to high protein binding and extensive tissue distribution.

Storage

Store the Tiova Inhaler capsules in the original blister packaging at room temperature (15–30°C), protected from moisture and light. Keep the HandiHaler device clean and dry; do not wash it. Keep out of reach of children and pets. Do not use capsules that are damaged, discolored, or past their expiration date.

Reviews

Clinical studies and patient reports indicate that the Tiova Inhaler is effective in improving lung function and reducing exacerbations in COPD patients. Many users report better symptom control and increased ability to perform daily activities. Some note the convenience of once-daily dosing, though a few mention dry mouth or throat irritation as minor drawbacks. Overall, it is regarded as a valuable maintenance therapy in COPD management protocols.
