Dosage and direction

Dosage must be individualized based on clinical response and tolerance. The typical starting dose for Wellbutrin SR is 150 mg once daily in the morning, which may be increased to 150 mg twice daily after at least 3 days. The maximum recommended dose is 400 mg per day, given as 200 mg twice daily. For Wellbutrin XL, the initial dose is 150 mg once daily, which may be increased to 300 mg once daily after several days. Doses should be administered at least 8 hours apart for SR formulation and taken whole, without crushing, chewing, or dividing. Dosage adjustments are necessary in patients with hepatic impairment.

Precautions

Patients should be monitored for emergence of anxiety, agitation, panic attacks, insomnia, irritability, hostility, aggressiveness, impulsivity, akathisia, hypomania, and mania. Use with caution in patients with a history of seizure disorder, cranial trauma, or conditions predisposing to seizures. May elevate blood pressure; regular monitoring recommended. Use cautiously in patients with hepatic or renal impairment. Abrupt discontinuation should generally be avoided.

Contraindications

Wellbutrin is contraindicated in patients with seizure disorder or history of seizures. Contraindicated in patients with current or prior diagnosis of bulimia or anorexia nervosa due to increased seizure risk. Should not be used concomitantly with monoamine oxidase inhibitors (MAOIs) or within 14 days of discontinuing MAOI treatment. Possible side effect

Common adverse reactions include dry mouth (up to 28%), nausea (up to 23%), insomnia (up to 22%), headache (up to 26%), and dizziness (up to 22%). Overdose

Overdose may lead to seizures, hallucinations, loss of consciousness, sinus tachycardia, and cardiac conduction disturbances. Fatal outcomes have been reported with massive overdoses. Symptoms generally appear within 4-8 hours post-ingestion. Management includes ECG monitoring for at least 48 hours, supportive care, and seizure control with benzodiazepines.
