Dosage and direction

  • Adults and Children 6 Years and Older: The recommended dose is 5 mg or 10 mg (1 or 2 tablets/chewables, or 5 mL or 10 mL of syrup) orally once daily, depending on symptom severity. Dosing may be initiated at 5 mg once daily for patients who may be sensitive to antihistamine effects.
  • Children 2 to 5 Years of Age: The recommended dose is 2.5 mg (½ tablet, ½ chewable tablet, or 2.5 mL syrup) orally once daily. The dose can be increased to a maximum of 5 mg per day, given as 2.5 mg every 12 hours.
  • Children 12 months to less than 2 years of Age: The recommended dose is 2.5 mg (2.5 mL syrup) orally once daily.
  • Children 6 to 11 months of Age: The recommended dose is 2.5 mg (2.5 mL syrup) orally once daily.
  • Administration: Can be taken with or without food. The tablet should be swallowed whole with a glass of water. For the liquid formulation, use the measuring device provided to ensure accurate dosing.

Precautions

  • While Zyrtec is less sedating than older antihistamines, patients should exercise caution when engaging in activities requiring mental alertness, such as driving or operating machinery, until they know how the medication affects them.
  • Use with caution in patients with pre-existing kidney impairment or hepatic impairment. Dose adjustment is recommended in patients with moderate to severe renal impairment (creatinine clearance ≤ 30 mL/min) or hepatic impairment.
  • Patients should avoid consuming alcohol or other central nervous system (CNS) depressants, as they may potentiate any sedative effects.
  • The chewable tablets contain phenylalanine (a component of aspartame) and should be used with caution in patients with phenylketonuria (PKU).

Contraindications

Zyrtec is contraindicated in patients:

  • With a known hypersensitivity to cetirizine, any of its inactive ingredients, or to hydroxyzine.
  • Who have experienced an allergic reaction to any piperazine derivative.

Possible side effect

Most side effects are mild to moderate and often transient. Commonly reported adverse reactions include:

  • Somnolence (drowsiness)
  • Fatigue
  • Dry mouth
  • Pharyngitis
  • Dizziness Less commonly reported side effects may include:
  • Headache
  • Nausea
  • Abdominal pain
  • Diarrhea
  • Agitation (primarily in children) Rare but serious side effects require immediate medical attention and may include severe allergic reactions (anaphylaxis), tachycardia (fast heart rate), and seizures.

Drug interaction

Formal drug interaction studies have shown no significant interactions with pseudoephedrine, azithromycin, ketoconazole, or erythromycin. However, the following should be considered:

  • CNS Depressants: The concomitant use of alcohol or other CNS depressants (e.g., barbiturates, benzodiazepines, opioid analgesics, antipsychotics, other sedating antihistamines) may potentiate the sedative effects of cetirizine.
  • Theophylline: High doses (400 mg once daily) of theophylline may slightly decrease the clearance of cetirizine, potentially increasing its levels. This is not expected with standard theophylline doses.

Missed dose

If a dose is missed, it should be taken as soon as remembered. However, if it is almost time for the next scheduled dose, the missed dose should be skipped, and the regular dosing schedule resumed. Do not take a double dose to make up for a missed one.

Overdose

In the event of an overdose (symptoms may include drowsiness, agitation, restlessness, and in large ingestions, tachycardia), symptomatic and supportive treatment is recommended. Gastric lavage may be considered shortly after ingestion. Cetirizine is not effectively removed by dialysis. The patient should seek immediate medical attention or contact a Poison Control Center.

Storage

  • Store at room temperature between 20°C to 25°C (68°F to 77°F).
  • Keep the medication in its original container, tightly closed, and protected from light and moisture.
  • Keep all medications out of the reach of children and pets.
  • Do not use after the expiration date printed on the packaging.
