Common use

Floxin is prescribed for moderate to severe bacterial infections including complicated urinary tract infections (pyelonephritis), bacterial prostatitis, community-acquired pneumonia, skin and skin structure infections, and sexually transmitted diseases caused by susceptible organisms. It is also used in ophthalmic formulations for bacterial conjunctivitis and in otic solutions for otitis externa. The medication is particularly valuable in treating infections caused by Pseudomonas aeruginosa, Escherichia coli, and other problematic gram-negative bacilli.

Dosage and direction

Adults (renal function normal):

  • Complicated UTI: 200-400 mg every 12 hours for 10-14 days
  • Respiratory infections: 400 mg every 12 hours for 10-14 days
  • Skin infections: 400 mg every 12 hours for 10-14 days
  • Bacterial prostatitis: 300 mg every 12 hours for 6 weeks

Renal impairment adjustment:

  • CrCl 20-50 mL/min: administer usual dose every 24 hours
  • CrCl <20 mL/min: administer half the usual dose every 24 hours

Take tablets with a full glass of water, at least 2 hours before or 2 hours after meals containing dairy products, antacids, or iron supplements. Complete the full course of therapy even if symptoms improve earlier.

Precautions

  • Monitor for tendon inflammation or rupture, especially in elderly patients and those taking corticosteroids
  • Assess neurological status regularly; discontinue if peripheral neuropathy occurs
  • Use with caution in patients with known or suspected CNS disorders (e.g., epilepsy)
  • Avoid excessive sunlight exposure due to photosensitivity risk
  • Monitor blood glucose levels in diabetic patients

Contraindications

Missed dose

Take the missed dose as soon as remembered, unless it is almost time for the next scheduled dose. Do not double the dose to make up for the missed one. Maintain the regular dosing schedule. If multiple doses are missed, contact the prescribing physician for guidance on resumption of therapy.

Overdose

Symptoms may include nausea, vomiting, dizziness, seizures, and mental status changes. Management involves gastric lavage if presented early, followed by supportive care including ECG monitoring for QT prolongation. Hemodialysis removes approximately 20-30% of the drug. There is no specific antidote; treatment should focus on symptomatic management and monitoring for neurological and cardiac abnormalities for at least 24 hours.
